In-Home Support

In-Home Support Services

In-Home Support services assist eligible, functionally impaired elderly persons age 60 and over in living dignified and reasonably independent lives in their own homes or in the homes of relatives or "caregivers" through the provision and coordination of various community-based services.

The variety of programs remove individual and social barriers to personal independence, empowers recipients with knowledge of personal care best practices, provides a continuum of care, and secures the opportunity to receive managed in-home and community-based long-term care services.

Aging-in-place is a term used to describe a person's ability to remain living at "home" as one age for as long, and as independently, as possible. When thinking about aging-in-place, it is important to consider the person's ability to manage their Activities of Daily Living (ADLs). ADLs are the tasks and routines that are performed on a daily basis in order to take care of one's self and live independently.
